Official ESA Letter
Property managers: start here. Cross-check Yudelmy Castaneda Sanchez, APRN on Florida MQA (license APRN9355049) and on the CMS NPI Registry (1730630799) against the English clinician PDF, then continue on /landlord-support. This page is not a registry and does not approve housing. Acceptance is never guaranteed under FHA + Fla. Stat. § 760.27. Housing only.
A landlord can check a Florida housing ESA letter without calling us.
The clinician’s license number and the lookup link are printed in the letter’s footer. Look up the clinician at flhealthsource.gov (MQA license verification). Enter the name and license number from the letter. Confirm the license is active and the type matches what the letter states.
What a valid letter includes: the clinician’s name and license type, the Florida license number, the date of the letter, a statement that the person has a disability-related need for the animal, and the clinician’s signature on letterhead. It does not need to name a diagnosis.
What is not enough: an online registration, ID card, certificate, or vest. Florida Statute § 760.27 says those are not, by themselves, reliable proof.
Housing only. The letter is for a reasonable accommodation in housing under the Fair Housing Act and Fla. Stat. § 760.27. It is not a registry and it is not a public-access document.
If the license does not match or is not active, the letter is not reliable. Ask the tenant for a current letter from a clinician whose license verifies.
The letter’s footer has the clinician’s name, license number, and a link to flhealthsource.gov (MQA license verification). Search there to confirm the license is active.
A registration, certificate, ID card, or vest is not reliable proof under Florida Statute § 760.27. Ask for a letter from a licensed clinician whose Florida license verifies.
No. It is for housing only. It is not a public-access document and it is not an airline letter.

Fair Housing and Florida Statute 760.27 let you request reliable documentation of a disability-related need. They do not require you to accept every letter, and a registry certificate is not enough on its own. Cross-check MQA + NPI with the links above, then continue on /landlord-support, do not demand diagnosis, severity, or full medical records when they are not needed. Not a registry. Acceptance is never guaranteed. This page is informational, not legal advice.
