Letters are structured so a housing provider can see: who the patient is, that a licensed professional has a relationship involving health care or disability-related services, that there is a disability-related need, which animal(s) are involved, and how to verify the clinician — without dumping unnecessary diagnostic detail into the landlord’s file.
