A Florida housing packet usually holds the lease, the association animal rule if any, the tenant’s written reasonable-accommodation request, and a clinician-signed letter naming Yudelmy Castaneda Sanchez, APRN (Florida license APRN9355049 · NPI 1730630799). Look the license up on Florida MQA. Confirm the PDF on /verify without a login, that verify page is the cross-check for the same English letter in the resident file. A vest, laminated ID, or internet registry number is not a substitute for that letter under Florida Statute §760.27. Board calendars and management portals still want the same English PDF. When the disability-related need is not apparent, you may request reliable documentation of disability and of the need for the animal. Ask for the unedited English clinician PDF plus a short written request that names the dwelling and the animal. Do not accept a filled-in sample from /sample-letter as authorization, that page is structure only. Keep the clinical letter English even when the tenant used Spanish or Creole on the site. Acceptance is never guaranteed. Not a registry.
